Sorry, I think I left a piece of code in there that was trying to redirect you to the development site where we did all the testing for the revamped look - that's why you were being challenged for a username and password.

I was just working through Lesson 11, and when I got to the Life Assessment, it appears that the follow-up questions halfway through Pornography and beyond are missing. There's just a lot of blank space to scroll through. Also, there isn't a "Submit" button anywhere on the page from what I can tell. I'm doing personal recovery without a coach, so I know that it's not entirely necessary for me to send in an assessment, but I still wanted to be able to do it. I know that the only personal screening I will ever need is "Are you engaging in behavior that is causing a disruption to your life or the lives of others?" and my answer right now is "Yes", but since I want to change and am making an effort to do so, I think submitting the Initial Life Assessment will be helpful.

Sorry for such a late response. The Assessment tools were designed by CoachJon and since his passing they have not been in use, nor monitored. They are more for your own personal reflection. With that, you do not need to have the entire form, as you can answer each of the questions as they pertain to each of the behaviours that you indicated in the previous section.
